The license is granted by the State Licensing Authority, operating under CDSCO. The procedure begins with filling out the MD 41 application form, which is filled out via the CDSCO SUGAM portal. For applying CDSCO MD 42, one will be required to submit the following documentation:
- Certificate of company incorporation
- Certificate of GST registration
- PAN card of the company
- Proof of business premises
- Rent agreement / Proof of ownership
- Proof of identity and address of directors/partners
- Proof of qualifications of technically competent personnel
- Proof of storage facilities
- Affidavits and declarations
